Nurse Disease Mgmt I

Location: Remote

Must have current RN license in the state of MN

The NurseDisease Mgmt I is responsible for participating in delivery of patient education and health/wellness interventions and for performing health coaching for members, across multiple lines, for health improvement/management programs. Primary duties may include, but are not limited to:

  • Conducts behavioral or clinicalassessments to identify individual member knowledge, skills and behavioralneeds.

  • Identifies and/or coordinatesspecific health coaching plan needs to address objectives and goalsidentified during assessments.

  • Interfaces with provider andother health professionals to coordinate health coaching plan for themember.

  • Implements and/or coordinatescoaching and/or care plans by educating members regarding clinical needsand facilitating referrals to health professionals.

  • Uses motivational interviewingto facilitate health behavior change.

  • Monitors and evaluateseffectiveness of interventions and/or health coaching plans and modifiesas needed.

  • Directs members to facilities,community agencies and appropriate provider/network.

  • Refers member to catastrophiccase management or higher levels of care.

  • Responsible for outreach,enrollment and management of Disease Management members. (Smokingcessation & weight management).

  • Provides education and healthcoaching resulting in measurable, improved health outcomes.

  • Conducts assessment, planning,implementation, coordination, monitoring and evaluation of members.

Specificsof role

  • Full time position (Monday - Friday40 hour work week).

  • You must have a current RNlicense in the state of MN.

  • This position will require youto obtain additional licenses and you must be willing to do this withassistance from the department

Hours of Position

  • Monday - Friday 8:30 AM to 5:30PM EST

Required:

  • AS/BS in nursing

  • 3 years of conditionspecific clinical or home health/discharge planning experience;

  • or any combination of educationand experience, which would provide an equivalent background.

  • Experience working withelectronic documentation and multiple databases.

  • Knowledge and experienceworking with Microsoft Office Products (Word, Excel, and Outlook)

  • 3 years of clinical RNexperience

  • Current unrestricted RN licensein current state you reside in.

Preferred:

  • Prior case managementexperience

  • Manage care experience

  • CCM strongly preferred

  • Previous work from homeexperience

  • Current unrestricted RN licensein state of MN
