Location: Shreveport/Bossier City LA - role will be based onsite at client location

Relocation bonus may be available for qualified applicants

Sign on bonus: $5,000

The Health Coach is responsible for providing health coaching to members and will serve as a resource to enhance the client wellness program

Primary duties may include, but are not limited to:

  • Conducts behavioral assessments to identify individual member knowledge, skills, and behavioral needs.

  • Coordinates specific health coaching to address objectives and goals as identified during assessment.

  • Implements coaching plan by using behavior change principles to identify member barriers and develop ways to overcome those barriers.

  • Coordinates with the interdisciplinary team to provide feedback on member goal attainment and clinical issues.

  • Monitors and evaluates the interventions and modifies

  • Delivers onsite, face-to-face lifestyle management programs and health risk reduction services sessions for individuals.

  • Triages medical issues.

  • Conducts group educational sessions on health topics of interest.

  • Onsite health coaching to reduce employee health risks by assisting employees in identifying and understanding health risks and then coaching them through the behavioral change process to reduce their risks

  • Develops, executes and provides support to specific client wellness program initiatives

  • Creates, promotes, and drives engagement through participation in programs and onsite wellness initiatives.

Qualifications

Minimum Requirements

  • Requires a BA/BS in appropriate field of specialization (examples such as Health Education, Exercise Physiology, Respiratory Therapy or Dietician) and 3 years of related experience in health education, exercise instruction, or patient education; or any combination of education and experience, which would provide an equivalent background.

  • Certification, advanced certification, and/or license appropriate to field of specialty required.

  • Some morning/evening availability required.

    Preferred Qualifications

  • Prior experience in health coaching, disease management and knowledge of behavioral and/or clinical assessment techniques, health and/or patient education and behavior change techniques preferred.

  • Working knowledge of wellness programming and health promotion

  • Understanding of disease management principles preferred.
