Transitions Health Coach

Work at home

Candidates MUST reside within 20 miles of Hamilton County or Boone County

Monday – Friday 8:00am to 4:30pm

Anthem is offering a new value added service, called care transitions, for Medicare members in the Hamilton and Boone County, IN area.

With the help of a Transitions Coach our members will be able to achieve goals they set for themselves and be better able to manage their chronic health conditions. The outcomes we anticipate are improved health and avoiding preventable hospitalization.

We are looking for personable, energetic professionals with a background in community based services, to include visits to client’s homes and collaboration with healthcare providers. Experience working from home is a plus.

In this role the Health Transitions Coach ® is responsible for providing skill transfer and guidance to the patient/caregiver for effective care transitions, improved self-management skills and enhanced patient-practitioner communication. Primary duties may include, but are not limited to:

  • Teaching members how to manage their medications, red flags of their conditions, and how to communicate effectively with treatment providers.

  • Conducts behavioral assessments to identify individual member knowledge, skills, and behavioral needs.

  • Coordinates specific health coaching to address objectives and goals as identified during assessment.

  • Implements coaching plan by using behavior change principles to identify member barriers and develop ways to overcome those barriers.


  • Requires a BA/BS in appropriate field of specialization (Social Work, Nursing, Health Education); 3 years of related experience; or any combination of education and experience, which would provide an equivalent background.

  • Ready access to reliable transportation is required from frequent travel to homes and hospitals.

  • Prior experience making home visits.

  • Must reside within 20 miles of Hamilton County or Boone County, IN area.

  • Travel reimbursement is provided.

  • Certification, advanced certification, and/or license appropriate to field of specialty as required.

  • Prior experience in health coaching, disease management and knowledge of behavioral and/or clinical assessment techniques, health and/or patient education and behavior change techniques preferred.

  • Understanding of disease management principles preferred.

  • Experience with discharge planning is preferred.

  • Excellent interpersonal skills, flexibility, and a passion for helping others are key for this position.

  • Bilingual is a plus.
